I cannot get NY Form IT-215 Claim for Earned Income Credit to generate even though it is required for filing with Form IT-2013 that is claiming the EIC.

I cannot get NY Form IT-215 Claim for Earned Income Credit to generate even though it is required for filing with Form IT-2013 if the taxpayer is claiming the EIC. There is no force to print and even if I hard input amounts into the fields, the form still doesn't show up until I check the Show All button.

How do I get this form to generate so that I can file it with the tax return?

Even if I were to paper file the NY return, I would still have to manually print the IT-215 to include it.
