Another option:
1) Buy a Delorean with a flux capacitor.
2) Channel 1.21 gigawatts into the flux capacitor to make time travel possible.
3) Go back in time to shortly before the form was deleted and tell yourself not to delete the file.
4) Return to the present time and the form will be there.
