CT Form CT-1065 D (a) and D (b) Generate a Statement

When Connecticut Form CT-1065 lines D (a) and D (b) are marked 'no' a statement is generated. However, line D (c) indicates a statement should be included when (a) and (b) are 'yes'.

This issue is scheduled to be resolved in a product update 04/30/2019 (Version 39.0430).
